DVM NEW PATIENT REFERRAL INSTRUCTIONS
Thank you for your referral to STVO. We work in conjunction with our referring DVMS so your clients can receive the best care possible at all times. We want this process to be convenient and efficient for DVMs and have created an easy online 3-Step process below.
STEP 1: SUBMIT NEW PATIENT REFERRAL FORM
- Fields marked with an * are required.
- Once Step 1 is completed, STVO will contact the pet owner directly to schedule their appointment.
- If you prefer, this online form can be downloaded as a PDF and sent via email along with pertinent medical records to reception@STVOpets.com.
STEP 2: EMAIL MEDICAL RECORDS
In addition to the online form, please send the patient’s current medical records, lab work, or any other related information VIA EMAIL to STVO prior to their appointment. Thank you.
STEP 3: ISSUE REFERRAL CARD
Provide the pet owner with a completed New Patient Referral Card. This card will serve as an reminder card and include STVO contact information along with a map and location address in preparation for their upcoming appointment.
